Intervención de enfermería en el paciente hematooncológico adulto con enfermedad injerto contra huésped aguda: a scoping review

Abstract

Objetivo: Mapear y sintetizar la evidencia científica relacionada con las intervenciones de enfermería en el paciente hematooncológico adulto con enfermedad injerto contra huésped aguda. Metodología: Se propuso realizar una scoping review que permitiera orientar a partir de la evidencia al profesional de enfermería acerca de las intervenciones en el paciente adulto con enfermedad injerto contra huésped aguda. La revisión se realizó en función del marco metodológico planteado por Arksey y O’Malley. Se llevó a cabo una búsqueda de literatura en las bases de datos MEDLINE, Embase y CINAHL, de lo cual se seleccionaron 27 artículos que cumplieron con los criterios de inclusión. Posteriormente, fueron sometidos a una evaluación critica mediante la aplicación de instrumentos de la Joanna Briggs Institute (JBI). Se obtuvieron 25 artículos que permitieron dar respuesta a los objetivos y proponer las intervenciones de enfermería a partir de la literatura consultada. Resultados: La tendencia temática se agrupó en los siguientes cuatro dominios de necesidades: Físico-fisiológicas; Cognitivas; Psicológico-emocionales; y Sociales. Acto seguido, se sintetizaron las intervenciones identificadas en la literatura consultada frente a las necesidades del paciente con enfermedad injerto contra huésped aguda. Conclusiones: La enfermedad injerto contra huésped aguda es la enfermedad que representa mayor riesgo de mortalidad en el paciente con trasplante de progenitores hematopoyéticos de tipo alogénico, pues afecta aspectos integrales. Por tal razón, identificar las necesidades e intervenciones soportadas y aplicadas de manera oportuna disminuyen el riesgo de complicaciones y muerte en el paciente, contribuyendo así a un mejor control de síntomas y calidad de vida (Texto tomado de la fuente).

Objective: To map and synthesize the scientific evidence related to nursing interventions in adult hemato-oncology patients with acute graft-versus-host disease. Methodology: A scoping review was proposed to guide the nursing professional based on the evidence regarding interventions in adult patients with acute graft-versus-host disease. The review was carried out using a methodological framework proposed by Arksey and O'Malley. A literature search was carried out in the MEDLINE, Embase and CINAHL databases, selecting 27 articles which met the inclusion criteria; later they were subjected to a critical evaluation through the application of instruments from the Joanna Briggs Institute. (JBI). 25 articles were obtained from which it was possible to respond to the objectives and propose nursing interventions based on the literature. Results: The thematic trend was grouped into four domains of needs: Physical/physiological, cognitive, psychological/emotional, and social. Subsequently, the interventions identified in the literature were synthesized against the needs of the patient with GvHD. Conclusions: Acute graft-versus-host disease is the disease that represents the greatest risk of mortality in patients with allogeneic hematopoietic stem cell transplantation, also delving into comprehensive aspects, therefore the identification of needs and injuries sustained and applied in a timely manner, reducing the risk of complications and death in the patient, contributing at the same time to better control of symptoms and quality of life.
