Abstract
Background: In Colombia, the implementation of diagnosis and preventive treatment for latent tuberculosis infection in people living with HIV has been historically insufficient, with no cost-benefit economic evaluations to guide the choice among available strategies.
Objective: To evaluate the cost-benefit relationship, financial viability, and asymmetry of benefits across payers of three strategies: interferon-gamma release assay screening with isoniazid chemoprophylaxis (IGRA-INH), tuberculin skin test screening (PPD-INH), and universal chemoprophylaxis (QP), compared to current clinical practice, from the social, Ministry of Health (MSPS), and Health Benefit Plan Administrators (EAPB) perspectives.
Methods: Cost-benefit analysis using a nested predictive model (decision tree and Markov chain), 17-year horizon, and 5% discount rate. Parameters were derived from a systematic review of systematic reviews and national administrative sources. Net monetary benefit, incremental benefit-cost ratio, economic rate of return, asymmetry of benefit across payers (Gini index), and deterministic and probabilistic sensitivity analyses were estimated.
Results: QP showed the highest net monetary benefit from the social ($1,404,436 COP) and EAPB ($2,890,202 COP) perspectives. From the MSPS perspective, no strategy was profitable, except IGRA-INH compared to PPD-INH. IGRA-INH offered the highest incremental efficiency and the lowest asymmetry across payers, was financially viable, and maintained the highest cumulative probability of cost-benefit throughout the horizon. The probability of progression from LTBI to active TB was the main driver of uncertainty.
Conclusions: Under the evaluated assumptions, IGRA-INH represents the best balance between efficiency, financial equity, and sustainability for the Colombian health system, although it does not maximize absolute NMB. Progressive implementation in PLHIV is recommended, conditional on diagnostic infrastructure, price negotiation, and financial coresponsibility between MSPS and EAPB.
